Objective: To determine the level and dynamics of absolute risks of malignancies implemented during the post-accident period to 2013 inclusive among clean up workers of 1986-1987 participation, evacuees and residents of the most contaminated areas. Methods: epidemiological, statistical, mathematical and software. Information on radiation doses available only for a one third of all liquidators and liquidators, in whom the disease of malignant neoplasm (cancer) is registered. Among the documents available to the liquidators in Kyiv and Kyiv region this information does not exceed 2%. Information on the year of participation in clean up work is available for 89.7% of the study cohort members (liquidators). More Information on available dates of participation is available for 54% of the cohort members. The incidence of all forms of cancer exceeds the national level only in the group of liquidators of 1986-1987. (SIR = 107,2%, 95% confidence interval (CI): 105,2-109,2). In recent years (2006-2013) corresponding figures (102,8%, 95% CI: 100,0-105,6) do not differ from the national level. There is a substantial excess of the expected incidence of thyroid cancer among clean up workers - in 4,3 times among evacuees - in 3,8 times, among residents of contaminated territories - in 1.3 times. The incidence of cancer of lymphoid, hematopoietic and related tissues in liquidators of 1986-1987 and evacuees exceeded national levels, while in residents of contaminated areas the figures were lower. Marked increase was observed in the incidence of breast cancer only in women - clean up workers of 1986-1987. In 1994-2013 biennium SIR = 157,8%, 95% CI: 141,0-174,7. The results of the current study show that the thyroid cancer has the greatest reason to be attributed to ionizing radiation exposure among other certain forms.
