Komisarenko Institute of Endocrynology and Metabolism of the National Academy of Medical Science of Ukraine" popup.description2 Object of the work: status of immune and hematological systems in patients with differentiated thyroid cancer of different age, with distant metastases: children and adolescents (under 18 years), young adults (19 to 40 years), and older people (41 to 75 years) after radioactive iodine therapy. Aim of the work: assessment of hematological and immunological outcomes of radioiodine therapy in thyroid cancer patients of different age, with pulmonary metastases. Methods: radiometric, spectrophotometric, immunological, flow cytometry, ELISA. Theoretical and practical results: for the first time, a dynamic comprehensive study has been carried out, of the impact of radioactive iodine on peripheral blood leukocyte composition, phenotype of peripheral blood lymphocytes, cytokines’ production, and innate immunity factors in 257 patients with differentiated thyroid cancer. An analysis has been conducted, of immunological changes depending on the metastases, patients’ age and cumulative radioiodine activity; short-term and long-term effects of radioiodine therapy have been determined. New data have been obtained on the features and mechanisms of formation of immune and hematological system disorders in the course of radioiodine therapy. It has been established that patients with distant metastases after radioiodine therapy had a wider range of immune and hematological disorders. A relationship has been demonstrated between the extent of radiation damage and patients’ age and cumulative radioiodine activity. Novelty: a method has been developed and patented for assessing the radiosensitivity of peripheral blood lymphocyte subpopulations of patients with thyroid cancer receiving radioiodine therapy. Advantages over existing analogues: the use of the suggested method of assessing the radiosensitivity of subpopulations of lymphocytes in peripheral blood of patients with thyroid cancer after radioiodine therapy provides a more informative determination of radiosensitivity of lymphocytes of different immunophenotype, whose changes reflect the impact of radioiodine on immune system cells and point out the possibility of secondary immunodeficiency development, which determines the advisability of immunotherapy in the combined treatment of these patients in order to improve their quality of life.
