This work presents the results of a complex inspection of 127 patients with paranoid schizophrenia. Using clinical-psychopathological, psychometric, clinical-catamnestic and psychodiagnostic methods studied the interdependence of dynamics in different groups of symptoms (productive, negative and cognitive) characteristics and expression of extrapyramidal symptoms and patients quality of life during the treatment with some classical and atypical neuroleptics, the role of cognitive impairment in clinical picture structuring. The results obtained in the study provide an opportunity for the differentiated approach to the treatment of patients with paranoid type of schizophrenia, depending on the structure of cognitive impairment. For patients with memory disorders - amisulpride has to be recommended, in order to improve the processes of switching the attention - amisulpride and risperidone, and to enhance concentration, intellectual ability and logical conceptual thinking - olanzapine has to be recommended.
