Objective: to increase the effectiveness of treatment of patients with uterine sarcomas by its individualization through a comprehensive evaluation of malignant potential tumors based on the immunohistochemical studies. Methods and apparatus: roentgenological, morphological, clinical, instrumental, statistical; universal dosimeter UNIDOS-Е. Theoretical and practical results: In this investigation was determined the significance of immunohistochemical markers depending on the histostructure of the tumor. In the absence of the marker Bcl 2 the recurrence of leiomyosarcoma was observed in 66.7 % of patients; in the absence of estrogen and progesterone receptors - in 80.0 % of cases; with a positive indicator of p53 protein expression - in 28.5 % of patients. At the same time 80.0 % of endometrial stromal sarcoma recurrence were observed in cases with a negative expression of estrogen and progesterone receptors. The expediency of radiotherapy in uterine sarcomas was substantiated, depending on their level and the histological forms of a tumor. It was proved that surgery and postoperative radiotherapy reduce the incidence of leiomyosarcoma manifestation in stage I-II against only surgery, especially when using intracavitary irradiation. There was no difference in terms of tumor progression in endometrial stromal sarcoma stage I-II, depending on the scheme. Novelty: was determined the significance of immunohistochemical markers p53, Ki-67, Bcl 2, ER, PR, etc., marker Bcl 2 and the presence of progesterone receptors are positive prognostic factors both in leiomyosarcoma and endometrial stromal sarcoma, especially with a negative expression of p53 protein. The effectiveness of implementation: exclusion of chemotherapy from a cancer patient's treatment plan results in a saving of medicines and money. Field of use: oncology, radiology.
