Objective: to improve diagnosing of the diastolic function disorders in the left ventricle of children with myocardium pathology (MP) by studying the type of structural and geometric myocardial alteration, adaptive resources of the cardiovascular system, features of the neurohumoral support, and analysis of the brain natriuretic peptide and proinflammatory cytokines concentrations. Methods: clinical; instrumental (ECG, echocardiography, Doppler echo-CG with isometric exercise, Holter BP and ECG monitoring, and the 6- minute walk test); radioisotope (angiotensin II, plasma renin activity, and aldosterone); biochemical (urine epinephrine and norepinephrine levels; immunofermental (CRP, Il-1 , Il-6, TNF- ). Results: diastolic function disorders are formed in 41.00% of adolescents with myocardial pathology. Diastolic dysfunction (DD) formation in adolescents with MP takes place on the background of the neurohumoral systems activation. Gradual rise of proinflammatory cytokines has been established in the study. The signs of tolerance disturbances regarding physical load and depletion of the adaptation reserves of the cardiovascular system have also been revealed. Novelty: formation of three types of the left ventricle DD have been found in adolescents with myocardial pathology. The criteria and an algorithm for diagnosing some early signs of DD in the left ventricle have been found in adolescents with MP. Effectiveness: the algorithm employment will contribute to improving the diagnosis of the earliest preclinical signs, regarding formation of diastolic filling disorders in the left ventricle at a latent stage of the disease.
