Objective: to determine the principal pathogenic mechanisms of MFD that occurred in the first year after menarche on the basis of retro- and prospective studies of hormonal and metabolic status in adolescent girls. Methods: clinical, immunofermental, biochemical, genetic, and statistical. Results: principal pathogenic mechanisms of MFD have been revealed during the first year after menarche and a concept regarding the possible role of moderate insulin resistance in their pathogenesis has been worked out. The risk factors that cause MFD in the first year after menarche have been established. The study has proved the dependence of the remote efficiency of the non-hormonal treatment of oligomenorrhea and secondary amenorrhea upon the initial hormonal background, and abnormal uterine bleeding upon the method of attaining the hemostasis in our patients. A high frequency of unfavorable remote consequences of MFD, which occurred during the first year after menarche, testifies to the necessity of seeking some new treatment methods for such patients and their timely referral to the counseling centers of reproductology. Novelty: the principal pathogenic mechanisms of MFD that occur in the first year of the MF formation have been established. They include: disorders of the gonadotropic pituitary function with elevated blood LH levels, increased insulin levels and free testosterone fractions as regards the findings in girls with regular menstruations. Effectiveness: the study made it possible to develop prognostic tables concerning the risk for MFD occurrence from the first year after menarche, which can be used not only by child gynecologists, but by some other specialists to define risk groups and to provide preventive measures.
