Objective: optimization of physical, sexual and mental development of adolescents with diffuse nontoxic goiter and improving the quality of their life by the development of systems for differential treatment. Methods: clinical, pathopsychological, ultrasound, genetic, immunofermental, immunological, biochemical and statistical. Results: the study has confirmed multifactorial nature of the diffuse nontoxic goiter: on its occurrence and course, besides iodine deficiency, influence hereditary and environmental factors, provision with microelements, first of all with selenium, and vitamins, as well as somatic and mental state of health. Novelty: the outcome of the study is a new concept regarding examination and treatment of patients with diffuse nontoxic goiter, based on determining the individual prognosis of the disease course and providing the use of differentiated therapy. Effectiveness: employment of the multimodal therapy in adolescents with an unfavorable prognosis of the disease course makes it possible to increase its effectiveness to 77.4% after 6 months and to 86.3% after 12 months of treatment. the algorithm employment will contribute to improving the diagnosis of the earliest preclinical signs, regarding formation of diastolic filling disorders in the left ventricle at a latent stage of the disease.
