Malaya Therapy Institute National Academy of Medical Sciences of Ukraine" popup.description2 The effectiveness of beta-blockers treatment in heart failure (HF) patients with type 2 diabetes mellitus based on the pharmacogenetic profile and markers of myocardial fibrosis was evaluated in our work. A total of 414 patients were examined. According to the study, the increased level of brain natriuretic peptide (BNP) was associated with an increase mass of the left ventricular myocardium, left atrium extension, increased levels of creatinine and galectin-3. The increased level of galectin-3 is associated with a favorable course of heart failure. The higher levels of serum galectin-3 and TNF were observed in HF patients with type 2 diabetes mellitus and the lowest in the patients without co-morbid endocrine pathology. Unfavorable course of HF was observed in the group of patients with Gly389Gly beta1-adrenergic receptors gene polymorphism. In these patients, there is a low chronotropic response to the bisoprolol use. In homozygous patients with mutant T-allele of G-protein beta3 subunit gene the use of carvedilol is more effective in the treatment of HF in respect of LVEF increase. In C-allele of G-protein ?3 subunit gene carriers the HF develops later, also they have lower level of monocytes and more often the deviation of blood creatinine from normal range.
