Objective: Treatment quality and results improvement in patients of different age categories with aortic valve disease by optimazing of surgical reconstruction techniques. Research methods: General clinical and special research methods. Aortic valve reconstructive surgery database, composed of 160 patients, has been analized. As a result the protocol of aortic valve preoperative assessment, indications and contraindications for reconstructive surgery in patients of different age groups has been developed. It was discovered that surgical valvuloplasty is an effective method of aortic stenosis treatment in neonates and infants with good immediate and mid-term results. 93.8% of operated patients remain alive in 5 year follow-up. During 5 year period after surgery only 6.2% of patients required reoperation on aortic valve. Aortic valve plasty is potentional surgical method of choice in older children. Previous interventions on aortic valve are not considered as an unfavorable predictors for aortic valve plasty. Bicuspid aortic valve - is an essential unfavorable factor for reconstructive surgery. Aortic valve replacement with autopericardial leaflets may become an alternative method of reconstructive surgery in cases with unfavorable aortic valve anatomy. Surgical approach needs further improvement and standardization. Considering generally poor prognosis and lack of specific conservative therapy, surgical correction remains substantial treatment method of aortic valve disease.
