Aim - improving the efficiency of the treatment of patients with bounded chemoresistant lung TB forms by developing and implementation of technology of surgical treatment of this contingent based on optimizing the use of surgical procedures.. Methods - clinical, surgical, radiological, laboratory. It was determined that the clinical efficacy of treatment of patients with limited forms of drug-resistant TB with surgical intervention was 98,2 % with the possibility to achieve a cure all of these patients. It was found that the optimal timing of surgery should be considered as a period of 7-12 months from the start of anti-TB therapy. It was established that the use of surgical treatment in patients with lung drug-resistant TB, besides the data bacteriological examination should be based on an assessment of the dynamics of clinical and radiological indicators of TB current. It is determined that all patients with TB recidive large residual TB changes in volume from 1-2 segments after the previous cases was observed; in 82,1 % patients with TB recidive and positive MBT culture the drug-resistance was observed, including: MDR - in 55,4 %, XDR - from 5.4 %; no less than the 65.2 % of the patients with TB recidive was appropriate to consider the use of surgery at the end of the previous case of the disease.
