The object of study - severe traumatic brain injury. Purpose - to explore the possibility of determining of the nervous tissue damage biomarkers proteins UCH-L1 and S100B in predicting the effects of a traumatic brain injury. This study is different in that it was carried out the theoretical generalization and practical clarification of the possibilities of using serum concentrations of biomarkers of nervous tissue damage in predicting the effects of severe traumatic brain injury (STBI) and diagnosis of pathophysiological features of traumatic brain injury. It is found that the biomarker of neuronal damage protein UCH-L1 and biomarker of astroglial damage protein S100B appear in the peripheral blood of patients with severe isolated STBI on the 1st day after injury, but the level of increase in their concentration with a high degree of sensitivity and specificity displays the effects of trauma, determined by scale effects Glasgow 6 months after hospitalization. It was shown that a comprehensive study of serum concentrations of biomarkers UCH-L1 and S100B, reflecting the ratio of glial and neuronal damage may contribute to the differential diagnosis of diffuse and focal brain damage. The study of biochemical biomarkers UCH-L1 and S100B are a more accurate tool for predicting the effects of TBI in comparison with traditional methods, such as the Glasgow Coma Scale and calculator forecast TBI IMPACT. Application area - neurotraumatology. Projected assumptions about the design development. Indicators of serum levels of the neurons damage biomarker protein UCH-L1 and astroglial damage biomarker protein S100B on the 1st day after STBI can be used to develop methods for express diagnostics of traumatic brain injuries, particularly in circumstances where performance of neuroimaging techniques isn't available (eg, in remote areas, disaster relief and military operations). Determination of TBI biomarkers can be an opportunity to diagnose severe brain damage at high risk of adverse effects, and to offer more intensive personalized therapeutic approaches and rehabilitation programs. Indicators of TBI biomarkers can become the part of new, more accurate, and improve the quality of existing mathematical methods of predicting the effects of traumatic brain injury (calculators).
