Methods: electron microscopy, visometry, spectral optical coherence tomography, rheoophthalmography, electroretinography, visual evoked potentials. Electron microscope studies of the optic nerve and retina conducted on 26 rabbits. We studied the morphological and morphometric changes in the retina in 43 patients with blunt ocular injury, morphometric features of choroid and choroidal blood circulation of uveal tract - in 20 patients, electroretinography and visual evoked potentials - in 20 patients. Electron microscopic studies revealed that the mild concussion eye changes in the structure of the optic nerve in terms of 1 week, 1 and 2 month. These changes primary concern ultrastructural damage of neurotubules and neurofilaments, mitochondria in axons of nerve fibers, the myelin sheath damage. Introduction of ceraxon intramuscularly reduces edema in these structures. In the early period after severe eye contusion we observed decrease of choroidal thickness in all sectors of the posterior pole and blood circulation of uveal tract according to ROG by 36.9% compared to the fellow eye. Visual acuity in the remote period after eye contusion depends on the morphological status of the outer retinal layers in fovea, established in the early period after trauma. It is more important to evaluate the morphological status of the external limiting membrane and the ellipsoid inner segments of photoreceptors in the fovea of injured eyes than morphometric retinal thickness-index in the central sector. There is the direct correlation between retinal thickness in the central sector and choroidal thickness in the temporal segment in severe structural changes in the fovea. The diagnostic algorithm for classification of the severity of the structural changes in the fovea according SOCT after eye contusion is based on an assessment of morphological changes in the retinal layers in the fovea. Pathological changes in the photoreceptor layer and bipolar and Muller cells identified by electron microscopy and electrophysiological studies are the basis for the appointment of neuroprotective and antiischemic therapy.
