Komisarenko Institute of Endocrynology and Metabolism of the National Academy of Medical Science of Ukraine" popup.description2 Object of the study: diagnostic, clinical, and therapeutic factors that influence life expectancy for young patients with thyroid cancer. Aim of the study: improve the methods of preoperative diagnosis and postoperative clinical follow-up regarding an increase in life expectancy for patients of young age with thyroid cancer. Methods of investigation: ultrasound, cytological, radiological, roentgenologic, cardiologic, laboratory methods. Results: it has been established ultrasound signs of regional lymph node malignancy, having been identified in 28 % of cases, and indications for repeat FNA; features of cell phenotype and thyrocyte nuclei asymmetry, which may be used in preoperative differential diagnosis of and prognosis of metastasis radio-resistance. A combined treatment (surgery, suppressive hormone therapy, radioiodine therapy) has allowed to achieve a complete remission of the disease in 90.3% of cases in patients with regional metastases and in 54.5% with distant metastases in thyroid cancer patients in the presence of using four courses of radioiodine therapy at a cumulative I-131 activity of 22 GBq. It has been established factors of cardiovascular risk, frequency and character of changes in reproductive organs of females of reproductive age based on USI data, which will allow to develop prevention and treatment measures in order to improve patients’ life quality. An algorithm has been developed, of diagnostic and therapeutic stages of a clinical protocol of management of patients with differentiated thyroid cancer after surgical treatment according to modern international protocols. Theoretical and practical novelty: the data obtained represent new and important information which will be taken into account in further studies on the identification of radioiodine-negative metastases in postoperative period.Novelty and advantages over analogues: the present study differs from current ones in an assessment of asymmetry of thyrocyte nuclei using computer morphometric analysis, which will be used for differential diagnosis of benign and malignant tumors. Efficacy of introduction: the data will be used to improve preoperative diagnosis and radioiodine therapy of metastases. Sphere of application: endocrinology, oncology, radiology,cardiology.
