Objective: Development and optimization of methods of intensive care at various critical conditions, intra- and postoperative analgesia techniques with regard to their cognitivesaving orientation. Methods: clinical, morphological, tissue metabolism, redox processes, indicators of the inflammatory response, biochemical markers of apoptosis. Results: improved treatment results, increased the speed of self-discharge of gases from 3,2±0,8 to 2,2±0,6 days, self-defecation - from 4,1±0,8 to 2,8±0,7 days and reduced the patient's stay in ICU from 4,0±0,3 to 2,8±0,3 days, reduced mortality by 26.1% and chance of die up to 8 times less frequently (OR = 8,667 [2,412; 31,141]) immediately after surgery intervention, at 6 months after surgery affect the quality of life of patients, reducing neurological deficit, which is confirmed by the absence of differences in stock likely to have minimal neurologic deficit (OR = 2,550 [0,392; 16,587]) with a decrease in postoperative mortality, and in the long term improved survival function up to 5 years of observation.
