The purpose of study: to improve the efficiency of radionuclide therapy of bone metastases by reducing the frequency and extent of complications by individualizing the selection regimen. Research methods and equipment: clinical, laboratory (determination of the level of white blood cells, hemoglobin, platelets, granulocytes), radiation, radionuclide 99mТс-Pyrphotech and 153Sm-oksabifor, dosimetric and statistical studies. Equipment: Gamma Camera, radiometer, X-ray machine, CT scanner, an automatic analyzer. Theoretical and practical results: The high degree of correlation between the introduced and the specific activity of the absorbed dose to the red bone marrow; between the absorbed dose to the red bone marrow and cumulative activity in the bone has been determined. In cancer patients with bone metastases defined the most efficient set of prognostic indicators of possible complications radionuclide therapy of bone metastases: the introduction of the specific activity; retained activity and volume of the trabecular lumbar vertebrae on the results of computed tomography images. A proposed method for selecting therapeutic activities 153Sm-oksabifor for subsequent courses of radionuclide therapy based on kinetics data of 153Sm-oksabifor in the body of patients and additional external radiometry in the first course of radionuclide therapy and CT studies. Recency: For the first time proved the absorbed dose to the red bone marrow during previous courses radionuclide therapy is important prognostic evaluation for the individualization of treatment for bone metastases. For the first time proved that introduced the specific activity, retained activity and volume of the trabecular lumbar vertebrae as a result of computer-tomographic images can be used as prognostic indicators of possible complications radionuclide therapy of bone metastases. The effectiveness of the implementation: Dosimetric planning of radionuclide therapy using trabecular volume L2-L4 lumbar vertebrae allows to individualize the assessment of the absorbed dose to the red bone marrow, which makes it possible to improve forecasting of the possible complications of the hematopoietic system.
