Aim of the work - to raise efficiency of treatment for the infected cutaneus radiation injuries by elaboration of a new method with usage of photodynamic therapy. Methods of investigation and equipment - general clinical, microbiological, morphological (histological, histochemical, morphometry), instrumental, biochemical, statistical. Spectrophotometer, photoelectrocolorimeter, coagulometer, microscopes, "TUR-60" X-ray device, "Barva-LED / 630" photon device.Theoretical and practical results - method of treatment for infected cutaneus radiation injuries with PDT was worked out. Experiments in vitro on cell cultures with S. aureus and P. аeruginosа optimal parameters of light-emitting diode (LED) irradiation and concentration of photosensitizer methylene blue were determined. Experiments in vivo on rats with radiation ulcers infected with microbes of different virulence (S. aureus, P. аeruginosа museum of clinical strains) allowed to assess efficiency of the elaborated PDT pattern. It was revealed that the number of necessary PDT sessions to achieve complete elimination of microbes depends on their virulence. The results of morphological study prove that antibacterial PDT activates considerably cleansing out necrotized tissues of the wound cavity and intensifies reparative processes up to complete healing of the sore. At systemic level antibacterial PDT favours normalizing hemopoiesis and hemostasis; reducing of peroxidation syndrome manifestations, activating of anti-oxidant enzymes, normalizing nitrogen oxide level, restorating of myeloperoxidase neutrophils activity. Novelty - for the first time a new method of treatment for infected cutaneus radiation injuries with photodynamic therapy has been worked out; PDT antibacterial effect on pathogenic microbes of varous virulence was assessed; complex morphological study of pathomorphosis of the infected radiation ulcer in rats while its treatment with PDT was carried out; it was established that on systemic level antibacterial PDT influences positively and normalizes condition of main homeostatic systems. Efficiency of introduction - the proposed method of treatment for infected cutaneus radiation injuries with photodynamic therapy allows to increase efficiency of the treatment and reduce significantly the terms of healing.
