Objective: to standardize the algorithms of topometric preparation and planning of conventional and conformal radiotherapy on linear accelerators for the treatment of malignant head and neck tumours and chest tumours. Methods and apparatus: radiological, morphological, clinical, instrumental, statistical; universal dosimeter UNIDOS-Е, linear accelerator Clinac 600 C. Theoretical and practical results: a complex of methods of pre-radiation preparation with recommendations for immobilization and ensuring of patient's comfort was suggested. Unified schemes of head and neck tumours and chest tumours irradiation were developed considering the "efficiency - risk" index and minimizing the exposure of risk organs. Have been perfected the approaches to optimal selection of three-dimensional or two-dimensional radiation treatment planning on linear accelerators of head and neck tumours and chest tumours depending on the constitutional characteristics of the patient based on the analysis of dose-volume histograms. Novelty: First developed standardized algorithms of topometric preparation and planning of conformal radiotherapy on linear accelerator for the treatment of malignant tumours of lung, head and neck, chest and prostate; methods of conformal radiotherapy planning were developed on the basis of calculation of the optimal plan coefficient and the radiation exposure of risk organs coefficient for the analysis of dose-volume histograms. New methods of pre-radiation preparation of patients with lung cancer and prostate cancer, and new scheme of chemoradiotherapy for localized cancer of the larynx were introduced. The effectiveness of implementation: the developed methods of conformal radiation therapy planning allow to optimize the process of pre-radiation preparation and radiation itself, and to increase the effectiveness of treatment of cancer patients.
