Objective: to increase the effectiveness of treatment of patients with uterine sarcomas by its individualization through a comprehensive evaluation of malignant potential tumors based on the immunohistochemical studies. Methods and apparatus: radiological, morphological, clinical, instrumental, statistical; universal dosimeter UNIDOS-Е. Theoretical and practical results: In this investigation was determined that Bcl 2 marker and the presence of progesterone receptors is a positive prognostic factor both in leiomyosarcoma and endometrial stromal sarcoma, especially with a negative index of p53 protein expression. In these patients the conduction of hormone therapy is expedient, and has contributed to prolonged remission in 90.0 % of patients. Chemotherapy prescription is recommended at a negative index of Bcl 2 expression and with negative progesterone receptors and positive p53 index. Determining individual levels of malignant tumor contributed to the development of the method of predicting recurrence in patients with uterine leiomyosarcomas with justification of treatment tactics. The expediency of using radiation therapy for uterine sarcoma tumors regardless of its histological form with the exception of G1 differentiation was substantiated. Novelty: complex evaluation of the malignant potential of tumors was firstly conducted based on the immunohistochemical analysis data, which depending on the indices of p53, Ki-67, Bcl 2, ER, PR, and other molecular biological markers allowed to adjust treatment regimens and improve its efficiency. The effectiveness of implementation: complex treatment of patients with uterine LMSs stage I-II possible to achieve long-term remission for 23 months, reducing the number of hospital days and cost of treatment.
