Aim of research: to establish the physiological, molecular, genetic and biochemical mechanisms of hypomethabolic state during the diabetes on the plains and in the midlemounting. Methods: The functional studies of the external breathing and gas exchange, cardiovascular system and oxygen transport (transthoracic impedance reopletyzmohrafiya, rheoencephalography, rytmocardiografiya, oksyhemometriya), electrophysiological, physiological; polarographic, biochemical, molecular biological, mathematical. It was showed reduce the severity of experimental diabetes in chronic hypoxia inhibition of energy metabolism in the rat liver mitochondria oxidation of NAD-dependent substrates with decreased enzyme activity of complexes I and III and disorganization respiratory electron transport chain. In highland the diabetes mellitus is accompanied with low changes in energy metabolism during oxidation FAD- and NAD-dependent mitochondrial substrates than on the plain. It was found the powerful defense mechanisms associated with hypoxic activation AMRK and apelin in the lungs and the myocardium of rats. In the adapting volunteers from the initial stages of diabetes found favorable impact highlans on the lipid and carbohydrate metabolism. In patients with diabetes highland residents shown the prevalence of hyperkinetic type of circulation. The results justify differences and mechanisms of hypometabolic state on the plain and in the highland. Criteria for therapeutic effects of hypoxia in diabetes elucidated, сlinical recommendations was given.
