Clinical manifestations and consequences of specific and nonspecific pathology are characterized for different types of twins. The rate of obstetric complications of pregnancy and labor was determined in patients with different types of twins, twins after IVF in particular, groups of highest perinatal risk were outlines. Methodology of advanced ultrasound exam in twin gestation was modified and tested, with consideration of typical and atypical manifestation of specific and nonspecific pathology and factors important for prognosis. A new methodology for determining discordance with the use of biometry of liver and thyroid gland was presented. Were defined ultrasound criteria of normal and abnormal development of twins in I, II and III trimester, were defined indications for referring the patient to a higher level institutions for further examination, further diagnosis and develop an individual plan for follow-up. There were characterized extrafetal structures (placenta, umbilical cord, amniotic fluid) and their interrelation with specific and non-specific pathologies. Risk factors for perinatal complications in different types of twins were determination and assessed, signs of the most severe forms of pathology were presented. Based on this data, there were developed different strategies of management of multiple pregnancy: a number of organizational, diagnostic, prophylactic and therapeutic measures, with individual approach to monitoring, treatment and delivery. Implementation of these measures helped to reduce the rate of complications of pregnancy and labor, especially the rate of preterm birth and to reduce perinatal losses.
