Material of study: blood cells (mononuclear and granulocytic white blood cells), blood serum. Objective: examine the relationship of immunological, genetic, hormonal mechanisms of the development of secondary systemic vasculitis and polyimmunopathologyunder conditions of systemic connective tissue diseases, and evaluate the effectiveness and safety of use of bioflavonoids and biguanides at that. Methods of the research: enzyme-linked immunosorbent assay, flow cytofluorometry, photocolorimetry, immune turbidimetry, spectrophotometry, genetic studies using the TagMan technique (expression level of miRNA-29b), statistical methods of study. The obtained results. In patients with vasculitis against the background of SCTD we have found a decrease of the grasping ability and reduction-oxidation activity of phagocytes, increased apoptosis, likely increase in CD4+/CD25+FoxP3+-, CD4+-, CD3+/HLA-DR+-, CD19+-lymphocytes, which makes it possible to predict the activity of clinical manifestations of the disease, the risk of vasculopathy formation, aggressiveness of cellular and antibody-dependent autoimmune processes. In patients with systemic sclerosis (SSc) with vasculitis and pneumofibrosis we have observed the increased level of MCP-1, TGF-? and IL4, reduced level of IFN-?, and with pulmonary hypertension - increased production of IFN-? and IL-17. The peculiarities of allergic anamnesis; clinical course, the level of total IgE, the number of B-and activated lymphocytes; titer of antineutrophilic cytoplasmic autoantibodies have been established for patients with systemic vasculitis (SV) with eosinophilia. It has been proved that miRNA29b biomarker showed high specificity and sensitivity in patients with SSc with pneumofibrosis against the backdrop of vasculitis. Immunological mathematical prognosis of the development of severe course of SLE and SSc against the background of vasculopathy has been developed. The application of Corvitin in patients with secondary vasculitis against the background of SCTD led to the likely reduction of CIC level, the increase in total serum complementary activity, stabilization of antiviral, humoral immunity and apoptotic activity of lymphocytes. The application of biguanides (metformin) led to a decrease in the expression on lymphocytesof apoptosis marker (CD95+), a slight inhibition of cellular autoaggression. Field of use: clinical immunology, rheumatology, allergology, angiology, nephrology, cardiology.
