Aim of SRW: to study epidemiology, risk factors, clinical-pathophysiological and neurbiologic mechanisms in pain syndromes occurring after ischemic or hemorrhagic stroke. Method of investigation: clinical, psychodiagnostic, neuroimagine, Ultrasound duplex scanning, biochemical, experimental methods. Results: Types of chronic pain after stroke were defined. It was found out, that post-stroke pain was formed irrespectively from size of the damage and stroke period, a little more frequently with location of the nidus in thalamus and in area of middle cerebral artery. Patients after stroke with pain syndromes had an increased thickness of intima media, their reaction of functional probes had a positive character. In these patients cognitive impairments were more frequent (76.6 % vs. 70.0 %) and raised to levels of mild-to-moderate disorders, whereas depressive problems were more manifested as compared with patients without post-stroke pain. A combination of chronic pain and depressive disorders in the post-stroke period resulted in formation of specific conditions based on "vicious circle", where pain and depression potentiate one another. Specific for pain syndrome formation and course biochemical markers (cathecholamines and p-substance) were investigated in post-stroke patients. Neurobiological mechanisms of post-stroke pain formation were studied in male rats. It was demonstrated the leading role of brain neocortical-thalamic mechanisms on the background of suppressive anti-nociceptive influences of central gray matter, that was accompanied by an increasing of serum dopamine levels and a decreasing of NO angioprotector properties.
