Aim of investigation: to study pathogenic mechanisms of formation of atypical forms of facial pains (prosopalgias), which are resistant to conservative therapy; to ground theoretically, experimental-morphologically, and clinically an integrated neurosurgical treatment for patients with atypical prosopalgias. Method of investigations: an analysis of literature sources; an experimental-morphological study of methods for therapy of prosopalgias; a clinical study of surgical methods for treatment of atypical pharmacologically resistant forms of prosopalgias. Results: It was found out that atypical pharmacologically resistant prosopalgias developed in damages of trigeminal, glossopharyngeal nerves, formations of sympathetic and parasympathetic systems of head and face. Groups of such prosopalgias were defined according to their location and etiology. Pathogenic mechanisms of pharmacologically resistant prosopalgias were studied. They contained an increasing of the nerve impulse flow in pain-conducted fibers with formation of nidi of pathological excitement in the central nervous system; a decreasing of the flow of pre-centre tactile and proprioceptive nervous impulses; damages of brainstem and thalamic formations. Benefits from cryosurgical and laser-surgical methods of treatment were demonstrated. On the base of these methods new techniques of treatment of prosopalgias with usage of Ukrainian-produced cryosurgical and laser-surgical devices as well as an integrated system of neurosurgical treatment for such patients were developed.
