Analysis of clinical and paraclinic progression of ulcerative colitis enabled us to single out the two phenotypes of the disease: chronic recurrent (favorable progression) and chronic continuous (unfavorable progression). The histological criteria of activity of inflammatory process in the intestines and the criteria of disease progression prognosis were developed for the first time for the children suffering from ulcerative colitis. It has been proved that the morphological substrate of unfavorable progression of the disease (continuous form) is the combination of morphological markers of high activity inflammatory process in the bowels with early chronicity sings. For the first time, the approaches to pathogenetic therapy were developed based on determining the disease phenotypes and progression in children with ulcerative colitis at early stages of examination. Diagnostics of the continuous form of ulcerative colitis in course of primary examination of the patient determine the relevancy of the combination therapy indications, which makes remission possible at earlier times. It has been evidenced that the quality of life of ulcerative colitis patients depends on the degree of inflammation severity, localization of the lesions, and morbidity.
