Objective: to optimize differential diagnostics and to improve the surgical treatment of patients with primary tumor lesions of the thoracic and lumbar spine. Methods of investigation - clinical, magnetic resonance and computer tomography, biochemical, experimental biomechanical, immunological, histological, mathematical modeling, statistical. Biochemical and immunological indices were studied in patients with primary tumor and metastatic lesions of the thoracic and lumbar spine and most characteristic changes were revealed. New and improved methods and devices for biopsy of spine tumors have been developed depending on the lesion density and their validation. On the basis of mathematical modeling, optimal variants of spondylodesis were determined for vertebroctomy of the thoracic and lumbar spine. On the basis of mathematical modeling, the use of various implants for vertebrectomy has been developed and substantiated. On the basis of experimental biomechanical studies, optimal variants of spondylodesis for vertebral vertebractions were determined. A new method and device for endoprosthetics of the spine segment has been developed. The indications for surgical treatment are improved depending on the level and extent of the lesion.
