The purpose of the work is to increase the efficiency of treatment of patients with oral cancer by developing functional and preserved approaches to complex treatment. Research methods and equipment: clinical, clinical-laboratory, rethenologic, pathomorphological, statistical, single-photon emission computer tomography "OFET". Complex treatment of 102 patients with resectable oral cancer of the III-IV stages was carried out according to the developed method with the use of the first stage of surgical intervention and postoperative simultaneous chemo-ray therapy. New and improved existing methods of plastic replacement of postoperative defects of the oral cavity with the use of regional and free shreds have been developed. The optimal reconstructive methods of elimination of postoperative defects of the oral cavity have been determined taking into account defect type, size and metastatic lesion of regional lymph nodes. The obtained results showed that carrying out of surgical treatment with reconstruction in the first stage allowed to achieve a minimum frequency of postoperative complications. Thus, complications after shank transplant were observed in 5.0% of patients, postoperative complications - in 6.0% of patients. The use of simultaneous chemo-radiation therapy in the complex treatment of patients with oral cancer decreases the duration of treatment for 3-4 weeks, depending on the number of courses of induction chemotherapy, prevents the emergence of cases of progression of the disease in the ineffectiveness of induction chemotherapy.
