The purpose of the work is to increase the effectiveness of treating patients with metastatic thrice negative and luminal Her2 / neu negative breast cancer by optimizing chemotherapeutic treatment, taking into account biological histochemistry of the tumor. Research methods and equipment: clinical, laboratory, instrumental, statistical; Ultrasound apparatus "Aloca", hematological analyzer ВС-3000, biochemical analyzer "Vitros", spectrophotometer "Thermo", centrifuge high-speed "Biofuge", thermostat "Termit". As a result of the conducted studies, it was determined that the expressed lymphoid cell infiltration of the tumor stomata was more often observed in a group of patients with three times negative RHZ (p = 0.00002). It was investigated that the groups of patients with RHZ differed both in the ratio of different sub-populations of T-lymphocytes and in the level of Ki-67: in the group of patients with TNRHZ, the average level of the KI-67 index was higher (50,76 ± 28,79%) compared with Zminalnym In the type (12,89 ± 16,09% p = 0,000001). A direct correlation between the number of stromal CD8 + lymphocytes and the level of Kji67 (r = 0,4, p <0,05), as well as the correlation correlation between expression of estrogen receptors and the number of CD4 + lymphocytes (r = -0,4, p <0.05) and the level of Ki-67 (r = -0.6, p <0.05) in patients with RH. Considerable deficiency of T-lymphocytes (in particular, cytotoxic T-cells) in peripheral blood of patients of both groups under study and significantly lower T-helper level in patients with TNRHZ were investigated. It was determined that the content of lymphocytes, which inherent regulatory properties tended to decrease. In patients with TNRHZ lymphocytopenia is due to insufficiency of natural killer cells and B-lymphocytes. It was investigated that in patients with luminal BG, the genotype GA of the TLR2 gene (G753A) was more often observed (RR = 3.08; 1.0? 5.27; p <0.05).
