The purpose of the work is to develop an algorithm for postoperative management of patients with iodine-negative metastases of differentiated thyroid cancer. Research methods and equipment - scintigraphy of the entire body with 99mTc-MIBI; whole body scintigraphy with 99mTc-DMCA; X-ray computer tomography; positron emission tomography with 18F-FDH; methods of statistical processing of results; two-detector gamma camera of the company "Mediso" (Hungary), single-photon emission computer tomography (EFEKT) "E. CAM 180". The performed studies confirmed the possibility of using techniques with non-ionic RPPs (99mTc-MIBI, 99mTc-DMCA) to detect iodine-negative metastases of the DRS. When performing a radionuclide study with 99mTc-pertechnetate with scintigraphic features of iodine-negative metastases of the DRS in the lymph nodes and mediastinum, there are cells of hyperfixation of radiopharmaceuticals in the tissue phase of the study, which are manifested as hypervascular in the angiographic phase. For the dynamic activity-time curves, which are built from the salivary glands, is a characteristic rising type with partial decline in activity, due to the secretion of saliva. This feature of the pharmacokinetics of 99mTc-pertechnetate allows differentiating the cells of hyperfection in the salivary glands and metastatically affected lymph nodes. PET / CT with 18F-FDH is a highly informative method for detecting iodine-negative metastases of the DRS with prolongation of the process in the lungs.
