Yanovsky popup.description2 Object - peripheral blood of 59 patients with chronic obstructive pulmonary disease (COPD) in remission, 27 patients with chronic bronchitis (ХБ) in remission, 17 healthy persons. Aim - to clarify the role of T-helper type 17 in the pathogenesis of chronic obstructive pulmonary disease and their participation in the formation of systemic inflammation in patients with this disease. Methods - clinical, immunological, statistics. New data on the participation of T-helper type 17 in the pathogenesis of COPD have been obtained - the production of IL-17A increases as the disease progresses, the frequency of exacerbations increases in a year and the respiratory function worsens. Increased blood levels of IL-17A are associated with inflammation rates (leukocytosis and neutrophiocytosis, high levels of TNF?, IL-8, C-RP in serum). Chronic inflammatory process in patients with CB by immunological signs is less intensive and in most patients is not accompanied by hyperproduction of IL-17A. A technology for detecting of concomitant cardiovascular pathology in patients with COPD has been developed, which consists in determining the level of C-RP in the blood serum of patients: an increase in its level 3.5 times or more is a marker of cardiovascular diseases. A technology for detecting activity of inflammatory activity in patients with COPD and CB, which is to determine of the IL-17A and C-RP levels in the serum of patients has been developed. Increased IL-17A level to 1.2 times or higher and C-RP to 1.7 times or higher are markers of inflammatory activity. A method for determining the activity of the inflammatory process in patients with chronic obstructive pulmonary disease has been developed, which allows increasing accuracy of determining the activity of the inflammatory process by 35.5%. The concept of the role of T-helper type 17 in supporting chronic inflammation has been developed, according to which they become significant in the pathogenesis of COPD at later stages, contributing to the more severe course of COPD. The formation of systemic inflammation is facilitated by exacerbations of COPD, which are accompanied by the release of proinflammatory cytokines and endothelial factors into the blood, which enhances endothelial inflammatory processes.
