The unsatisfactory functioning of the antituberculous service for 2005-2016 and its shortcomings was shown, which work ineffectively and the need for its reorganization and restructuring. The need to reduce the bed capacity by 20,8 % and its restructuring was substantiated. The prognosis of incidence and mortality from tuberculosis (TB) for 10 years (until 2025) was developed. The main causes that influence on the TB incidence and mortality were identified. Assessment of the results of the implementation of the "National Program for Countering the Tuberculosis Disease in 2007-2011" and the program in 2012-2016 showed insufficient monitoring of TB, the goals recommended by WHO to reduce the incidence and mortality from TB have not been achieved. The main evaluation predictive indicators of the effectiveness of measures for idetecting, diagnosis, treating TB patients and preventing of the disease had been identified, which ensures the control of the TB epidemic in Ukraine. The method of prognosis of the incidence and mortality from TB was substantiated based on the modeling of the processes of their dynamics by the method of a polynomial two-step trend, as well as the criteria for evaluating the rational use of hospital beds in TB dispensaries, hospitals in sanatoriums. The Conception of the TB service reforming in Ukraine was developed on the basis of the reform of the health care system, a draft order of the Ministry of Health of Ukraine was prepared.
