G. Yanovsky popup.description2 Object: 1279 patients with MDR TB. Aim: to determine the optimal chemotherapy regimens for patients with mono- and poly-resistence, RifTB, MDR and XDR TB depending on the number, amount of anti-TB drugs at the treatment regimen, the length of intensive and continuation phases of chemotherapy, which will increase the treatment efficasy and save public funds. Methods: clinical, radiological, laboratory, microbiological. A technology for the use of a 12-month abbreviation for antimycobacterial therapy in patients with multidrug-resistant TB has been developed, which allows, compared to the 20-month standard course, to improve the efficacy of treatment in patients with new cases of multi-resistant tuberculosis without resistance to aminoglycosides / polypeptides at the end of the main course of treatment with 69.8 % to 95.4% against the background of equally satisfactory tolerability of treatment, which reduces the total cost of treatment by 1.5 times among patients without resistance at fluoroquinolone or aminoglycoside / polypeptide and 2.4 times among patients with this resistance. The use of the 12-month reduced chemotherapy regimen compared to the standard 20-month course can increase the rate of "effective treatment" in patients with new cases of multi-resistant pulmonary tuberculosis with resistance to fluoroquinolones or aminoglycosides / polypeptides at the end of the course almost 2-fold ( from 46,9 % to 90,6 %). The effectiveness of the 12-month regimen at the end of the treatment compared to the end of the intensive phase was almost non-existent (90,6 % "effective treatment" and 96,9 % "bacterial withdrawal").
