Aim - to increase the effectiveness of treatment of patients with acute community-acquired infections of the lower respiratory tract (ACAILRT) through optimization of etiological diagnosis and differential application of antimicrobial chemotherapy taking into account pharmacoeconomic aspects. Methods - clinical, radiological, microbiological, statistics. A new algorithm of etiological diagnostics of ACAILRT was developed on the basis of a combination of classical bacteriological methods, "fast" immunochromatographic tests and a multiplex polymerase chain reaction in real time, which allowed to establish etiological agent in 86.8% of patients. New data on the prevalence and range of pathogens of GNIIDS were obtained: viral pathogens were identified in 64.1% of cases, in 22.8% - bacterial, and in 13% - their associations.
