Objective: To improve the effectiveness of treatment (preservation of working capacity and quality of life improvement) in patients with systemic rheumatic diseases by improving diagnostics, early start of pathophysiologically grounded therapy and prevention of complications. Methods of research - general clinical, evaluation of articular status, biochemical, serological, complex indices (determination of activity indices and response to treatment), instrumental (ultrasound examination of knee joints). Established: The level of elastin with high sensitivity (75%) and specificity (89%) allows differentiation of patients with PN and ANCA-associated vasculitis. The level of alpha-actin-2 may be a promising candidate for an assessment of the response in patients with primary systemic vasculitis (PSV). The possibility of BAFF level use as an objective criterion for evaluating the activity of the PSV is demonstrated. The possibility and advantages of using the level of matrix metalloproteinase-3 as an objective criterion for the effectiveness of PsA treatment is demonstrated. The high therapeutic efficacy of leflunomide has been demonstrated not only in terms of joint damage, but also in relation to psoriatic skin lesions. It is suggested to use predictors of clinical response to leflunomide therapy in patients with PsA. The use of avocado oil and soybean unsaponifiables has been shown to result in clinically significant improvement with positive changes in cartilage metabolism and its ultrasound characteristics in patients with knee osteoarthrosis.
