V., Registration Date 06-03-2018 Organization National Medical Academy of Post-Graduate Education named after P.L.Shupyk popup.description2 The study was focused on a high frequency of obstetric and perinatal complications, cases of maternal and perinatal mortality in HIV-infected women with active tuberculosis (TB). According to the results of the study, there were identified risk factors for tuberculosis in HIV-infected pregnant women. Based on the identified factors, a questionnaire for HIV-infected pregnant women was developed to reveal a high-risk group for TB during pregnancy. There were developed the algorithms of management of HIV-infected pregnant women with active pulmonary tuberculosis, and HIV-infected pregnant women whose pregnancy came against the background of residual changes following tuberculosis. There were developed and implemented the treatment-prophylactic methods of pregnancy follow-up in HIV-infected women with residual changes following pulmonary tuberculosis, the use of which will promote the reduced frequency of obstetric and perinatal complications. An integral analysis of the obtained results allowed identifying the risk factors of perinatal pathology associated with HIV and herpesvirus infection. Infection activity indices, the condition of the immune system, hemostasis, vascular regulation, microbiota, fetoplacental complex were found to be among the most significant. The identified risk criteria became the basis for developing a prediction algorithm for obstetric and perinatal complications. The established links of pathogenesis created a theoretical basis for the development of a pathogenetically directed system of perinatal fetal protection in HIV-infected women with associated herpesvirus infection, including the stages of additional testing, prediction of the risk of complications and a differentiated set of therapeutic measures aimed at eliminating the revealed impairments (antiviral, immunomodulatory, detoxification , endothelial-protective effect, normalization of biocenosis, restoration of local mucosal immunity). The benefits from the research include the reduced frequency of obstetric and perinatal complications in HIV-associated infections in women, the decreased mortality rate in women with co-infected HIV and tuberculosis, the reduced rate of mother-to-child transmission of HIV and the incidence of utero infection, the reduced HIV incidence, the reduced mortality rate of HIV-infected women, and the reduced level of disability due to perinatal causes.
