The purpose of the study is to create a scientific basis for the system of prognostication and prevention of perinatal and obstetric complications for increasing the effectiveness of obstetric hospitals, including perinatal centers. Research methods: anthropometric, clinical, biochemical, ultrasound, immuno-enzyme, molecular-genetic, statistical. Theoretical results: as a result of a comprehensive genetic study of pregnant women with overweight or obesity for carrying the genes responsible for the exchange of folate cycle and thrombophilia genes, the main candidate genes associated with the body mass index were identified. Novelty. The need to identify the form of thrombophilic disorders at the planning stage of the procedure for ovarian stimulation in women who are planning to use assisted reproductive technologies for the first time or have already failed during previous attempts has been proved in order to increase the efficiency of implantation. This made it possible to individualize therapy and reduce the risks of gestational complications and perinatal losses. The significance of the results is to create an individual approach to the diagnosis, prevention and treatment of thrombotic complications in women with thrombophilia, which led to an improvement in perinatal outcomes in pregnant women with hereditary and acquired forms of thrombophilia due to timely early therapy. Efficiency of implementation. Medical efficiency - improving reproductive health and increasing the birthrate of healthy children in women of high perinatal risk in obstetric hospitals at secondary and tertiary levels. Social effectiveness - the prevention of disability from childhood in preterm birth and multiple pregnancy. Economic efficiency is a decrease in the cost of treatment in 1,5-2 times due to prolongation of pregnancy in preterm delivery and reduction in the timing of nursing of deep-seated children, which is especially important for socially unprotected layers of the population, which is one of the priority medical and social problems of Ukraine.
