Objective: broaden the understanding of modern pathogenetic mechanisms of development of GERD and FD in adolescents based on the study of motor and evacuation function of esophagus and stomach, nature of acid formation, neurohormonal regulation, internal health pattern (IHP) taking into account the infection with Helicobacter pylori (HP). Methods: general clinical, psychological, biochemical, instrumental, immuno-enzymatic, respiratory test (sensory analysis). Results: features of clinical and morpho-functional manifestations of GERD and FD in adolescents were determined. It was proved that in 49.9% of patients GERD is accompanied with the development of reflux esophagitis (RE) which was diagnosed more often in children with a burdened antenatal history, was characterized by more severe course and more pronounced disregulatory disorders. The frequency and nature of changes of cortisol, melatonin and serotonin levels was determined, peculiarities of neuroendocrine regulation of patients with this pathology was determined, similar changes in their metabolism were observed in adolescents with FD and GERD without esophagitis. It was shown that GERD and FD in the vast majority of patients are accompanied with decreased motor-evacuation function of the stomach, its frequency and severity are more typical for FD. For the first time the peculiarities of IHP in adolescents with GERD and FD were determined, and the presence of insufficiently manifested behavioral, value and volitional components of IHP was demonstrated. For the first time the characteristics of the respiratory test (sensory analysis) of adolescents with GERD were determined, the associations between the individual parameters of exhaled gas and serotonin were shown in patients with GERD and FD. The technology of early screening diagnostics of RE is developed. Novelty: for the first time the presence of common features and differences between GERD and FD in adolescents is established, the significance of the different levels of regulation is substantiated and certain mechanisms of development of these diseases are revealed, the most important factors influencing their course are determined. For the first time, the psychological characteristics of adolescence IHP were presented and the parameters of exhaled gases of adolescents with GERD using the sensory devices were analyzed. Efficiency: the introduction of research results would improve the methods of diagnosis, treatment and prevention of GERD and FD, increase the effectiveness of therapy and prevention of this pathology in adolescence.
