Purpose: To determine the peculiarities of adaptation and functional capacity of the cardiovascular system in children with arterial pressure disorders and secondary cardiomyopathies and prove the feasibility of physical activity for this category of children. Methods: Ruth’s test, pair test ‘6-minute walking’, clinoorthostatic test, heart rate variability, echocardioscopy at rest and after physical loading, bicycle ergometry, cortisol, serotonin and lactate content in blood, content of catecholamines in daily urine, statistical methods. Results: Frequency of reduction of tolerance to physical activity in adolescents who suffer from secondary cardiomyopathies and arterial hypertension has been determined. The content of cortisol, lactate and excretion of catecholamines in patients depending on exercise tolerance has been studied. The predictive value of parameters of the cardiovascular system, anthropometry data of patients, indicators of the sympathoadrenal system, cortisol, serotonin and lactate of blood for evaluation of physical activity tolerance have been determined. The volume of functional tests for the full assessment of the adaptation possibilities of the cardiovascular system and the possibility of physical activity for children with the specified pathology has been substantiated. Novelty: The need in comprehensive assessment of the adaptation possibilities of children with cardiovascular pathology, which includes determination of the state of vegetative support of activity, evaluation of the reaction of "rapid" adaptation and tolerance, has been justified for the first time. Prognostic significant signs of reducing the adaptation possibilities of children who suffer from secondary cardiomyopathies and arterial hypertension have been determined: tachycardia, hyperdiastolic variant of the clinoorthostatic test, hyperkinetic version of hemodynamics, low aerobic reserves, and the presence of comorbid pathology. Efficiency: implementation of this technology will allow adequate formation of groups of physical education in school; extension of physical load for children who suffer from secondary cardiomyopathies and arterial hypertension; improvement of rehabilitation of children with cardiovascular disorders. Field of application: pediatrics, family medicine, children's cardiorheumatology, rehabilitation.
