Study purpose - to analyze the anamnestic (thrombotic complications, transformation to acute leukemia), clinical hematological features and laboratory parameters in patients with Ph-negative MHN who were affected by the Chernobyl accident and in patients with spontaneous Ph-negative MHN. Study methods - clinical, hematological, molecular genetic, statistical. The frequency and spectrum of the most prevalent nosological forms have been studied among patients with Ionizing Radiation (IR)-associated and spontaneous Ph-negative MHNs. The mutation of the JAK2 gene was detected in patients with ionizin radiation associated and spontaneous Ph-negative MHNs. It was shown that the average value of hematocrit in exposed to IR-PV patients was higher than in spontaneous PV patients. The number of platelets in ET patients who were not affected by the Chernobyl accident was higher than in IR-associated ET patients. The frequency of vascular episodes was more common among Ph-negative MPN patients positive for JAK2 V617F mutation. Cases of Ph-negative MPNs transformation to acute leukemia were documented only among patients with PMF who are positive for the JAK2 V617F mutation. The frequency of the transformation to acute leukemia cases is higher among patients who experienced IR compared to those who were not exposed to IR. Thus, it was proved that inclusion of MPL, CALR gene mutations detection in the diagnostic algorithm of Ph-negative MPNs is reasonable for providing reliable and unified examination and determining the risk group of patients who were exposed to IR.
