Classification of pitutary adenoma extension to shpenoid sinus allows qualitative and quantitative determination of indications for the adaptation of surgical access. Posterior septostomy at Grade 2 and Grade 3 extension into the sphenoid sinus is necessary for safe endoscopic endonasal surgery. Intraoperative ultrasound dopplerography is an informative method that promotes a safe EET surgery pituitary surgry with laterosillar extension. All pituitary adenomas with cavernous sinus extension will require long-term lifelong multidisciplinary observation and integrated treatment using the capabilities of modern radiotherapy and radiosurgery.
