The aim of the work was to increase the treatment effectiveness at patients with severe forms of psoriasis by developing the complex therapeutic method based on the study of clinical, biochemical and immunological parameters. Methods: clinical, biochemical, immunofermental, instrumental, statistical. The data on the nature of the cytokine disbalance in psoriasis have been refined. Dependence of levels of interleukin-1beta, -10, highly sensitive C-reactive protein, epidermal growth factor on severity of dermatosis and arthritic syndrome activity were determined. The clinical and pathogenetic role of vasculoendothelial growth factor in the course of psoriasis is established. The clinical and pathogenetic role of indicators of stable metabolites of nitric oxide has been determined and their relationship with the prothrombin index, fibrinogen and soluble fibrin-monomeric complexes in patients with severe forms of psoriasis has been established. The direct dependence of the content of these indicators on the degree of severity of dermatosis and joint syndrome activity was established. The method of infrared remote thermography allows to determine inflammation in the joints and periarticular tissues at the early preclinical stage, to determine the degree of inflammation activity at the stage of marked changes and to monitor the therapy. The complex method of treatment at patients with severe forms of psoriasis with the use of basic therapy, microcirculation improver (L-arginine) and chondroprotector (glucosamine sulfate) is developed.
