G. Yanovsky popup.description2 Object - 202 patients with newly detected pulmonary sarcoidosis. Aim - to increase the effectiveness of treatment in patients with pulmonary sarcoidosis with resistance to glucocorticosteroids (GCS) or contraindications to corticosteroids use, based on optimal regimens, utilizing second line drugs. Methods - clinical, chest computed tomography, spirometry, diffusion lung capacity. There was developed an algorithm of personificated therapy of sarcoidosis patients, based on the registration of contraindications, serious adverse reactions resistance and responce to GCS therapy in each taken case. Use of this algorithm allowed to reach clinical cure in 97,0 % of sarcoidosis patients. The fatigue scale was offered in current category of patients as an additional criterion of evaluation of treatment effectiveness. There was developed the method of treatment of stage II-III pulmonary sarcoidosis patients at the presence of GCS-therapy contraindications, based on sequential use of methotrexate, which increases the effectiveness of therapy in patients with sarcoidosis up to 84,0 %.
