The aim of the work is to determine the optimal regimens of chemotherapy for patients with mono-, poly-, rifampicin, MDR and XDR tuberculosis to the composition, amount of anti-tuberculosis drugs, duration of intensive and supportive phases of chemotherapy, which will improve the efficiency of treatment of patients and save state funds. Methods: clinical, radiological, laboratory, microbiological statistical (retrospective and prospective). The technology of appointing algorithms for reduction of 12-month regimens of linezolid based chemotherapy in patients with new cases has been developed, which led to an increase in the effectiveness of treatment in patients with MDR - by 14,8 %, in patients with pre-XDR - by 41,6 % and reduced the cost of treatment by 1,5-2,4 times. A method for treating patients with the first-ever resistant rifampicin to pulmonary tuberculosis was included an additional appointment of linezolid to a standard regimen of chemotherapy with daily application in the average daily dose for one treatment of 5 effective anti-tuberculous drugs, the duration from the start of treatment and until receiving the MBT drug sensitivity test, and allowed to increase the rate of bacterial exclusion by 31,9 %, the disappearance of clinical manifestations by 16,3 %, to avoid progression against the background of the same transfer awens. The method of conducting adverse reactions in patients with MDR was developed, which included all active pharmacovigilance activities (adequate monitoring, treatment and registration of adverse reactions) that resulted in detected severe adverse reactions in less than 5,0 % of cases and reduced the frequency of treatment interruptions by 15,0 %.
