G. Yanovsky popup.description2 Оbject of the study - 461 patients with bronchial asthma. Aim - to increase the accuracy of asthma diagnosis taking into account the phenotypes of the disease. Methods - questionnaires, clinical, functional, X-ray, immunological, genetic, statistical. The basic clinical-functional, immunological, genetic and radiological features of the severe course of bronchial asthma (BA) have been established, which allowed to develop a technology for the diagnosis of severe bronchial asthma at the stage of primary medical care and the technology for diagnosing the phenotypes of severe bronchial asthma at the stage of specialized medical care. The algorithm of diagnostics, management and prediction of severe bronchial asthma taking into account the phenotypes of the disease, which allows at the primary care level to increase the accuracy of diagnosis of severe asthma by 20,0 % and to identify the likely phenotypes of the disease. At the specialized medical care level it is possible to allocate an additional phenotype of asthma with fixed bronchoconstriction in 14,4 % among of the patients with a possible combination of BA and chronic obstructive pulmonary disease (COPD) and to improve the accuracy of diagnosis allergic asthma by 10,0 % (with allergic rhinitis by 8,8 %, polyposis ethmoiditis by 5,6 %), non-allergic asthma by 10,0 %, BA associated with COPD by 14,4 %, asthma with concomitant diseases - the cardiovascular diseases by 23,2 %, sleep obstructive apnea-hypnea syndrome by 10,6 %, followed by the prognosis of the severe course of asthma with regard to the disease phenotypes. The technology of the increased airway resistance diagnostics in patients with bronchial asthma, chronic obstructive pulmonary disease and their combination was developed, the application of which allows to improve the accuracy of the diagnosis by 10,0%. A new method of the small airway diagnostics obstruction in patients with bronchial asthma was developed, which allows to increase the diagnostic accuracy by 22,0 %.
