Aim - to increase the efficiency of treatment of patients with limited forms of HRTB of the lung through the development and implementation of the technology of surgical treatment of this contingent by optimizing the use of surgical interventions. Methods - clinical, surgical, radiographic, laboratory. A technology has been developed for the surgical treatment of patients with limited forms of HRTB, which provides a reasonable sequence of actions to form a contingent of patients with indications for surgical treatment, determine the timing, scope of examination and preoperative preparation, type of surgical intervention and its technique, clarify the postoperative chemotherapy regimen based on resection material. The use of the development made it possible to achieve a cure rate of 96.7 % of patients, decrease of relapses number to 1.8 % and no lethality. It has been established that in the treatment of patients with HRTB using surgical intervention, there is a tendency to an increase in direct medical costs by 9.2% with a significant decrease in the cost/effectiveness ratio by 210.2 %. The developed method of atypical lung resection in patients with limited forms of HRTB is provided a significant reduction in the duration of the operation from 27.5 minutes to 9.2 minutes, a decrease in the number of postoperative pulmonary and pleural complications and the repeated operations by 96.4 %, a reduction in the period of postoperative inpatient treatment from 20.7 days to 14.5 days.
