Aim - to increase the effectiveness of treatment of patients with acute community-acquired infections of the lower respiratory tract (ACAILRT) through optimization of etiological diagnosis and differential application of antimicrobial chemotherapy taking into account pharmacoeconomic aspects. Methods - clinical, radiological, microbiological, statistics. The seasonal prevalence and the spectrum of the pathogens of ACAILRT in 88,8 % of patients wаs obtained - in 64,5 % of cases identified viral pathogens, 24,4 % - bacterial, 10,1 % - of their association. Developed the scheme of treatment of patients with CAP of viral and bacterial etiology through the additional use of the antiviral drug vitaglutam reliably reduce by 47,0 % the incidence of infectious complications and the duration of their occurrence, the duration of antibiotic therapy by 2.4 days, and in combination with the combined drug arginine hydrochloride and levocarnitine - reduce the duration of intoxication syndrome by 2 days and the duration of treatment by 3.8 days due to positive changes of free radical peroxidation of lipids.
