Purpose: to study the formation of vascular pathology in patients with RA based on the assesssment of metabolic disorders (dyslipidemia, hyperhomocysteinemia, vitamin status, paraoxonase activity, inflammation markers, adipocyn levels), genetic (genetic polymorphism by MTHFR, promoter of the NOS3 T786C gene (rs2070744), disorders in the hemostasis system and their associationt with the course of the disease and the state of the cardiovascular system, to identify the most adverse combination of metabolic risk factors and develop approaches to there pharmacological correction. Methods of research: clinical examination of patients, biochemical research, research of hemocoagulation system and platelet aggregation, genetic research, instrumental research. The functional state of the vascular endothelium and morpho-functional state of the myocardium was studied in rheumatoid arthritis patients depending on age, sex, duration of disease, injury index and inflammatory activity. The endothelial dysfunction and carotid ultrasound indicators of arterial was assessed depending on the content of proinflammatory cytokines and cell adhesion molecules in serum; features of lipid metabolism disorders were evaluated in RA patients and in the control group; the activity of serum paroxonase its association with lipid metabolism disorders, atherosclerotic lesion of blood vessels and structural and functional state of the heart was determined; the frequency of hyperhomocysteinemia and mechanisms of its formation was evaluated. Also significant leptin increase and adiponectin reduction was detected compared to control group, indicating there possible pathogenic role in RA. Stratification of the individual absolute risk of ischemic events has been stratified and its relationship with the course of the disease has been assessed. The preventive measures were designed aimed to correct the cardiovascular risk factors that will not only reduce the frequency of manifestations of atherosclerotic vascular lesions but also increase the rehabilitation potential in patients with clinical signs of vascular lesions.
