It has been established that osteoporosis leads to hypocalcemia, hypophosphatemia, increases the activity of total alkaline phosphatase and its isoenzymes, as well as the activity of serum acid phosphatase. Pathological changes occur in the structure of compact bone tissue and epiphyseal cartilage. Prolonged administration of prednisolone is characterized by a significant decrease in the expression of vitamin D3 receptor (VDR) in bone tissue and significant changes in the functioning of the NF-?B-associated cytokine system RANKL/RANK/OPG, which indicates a slowdown in the remodeling process, the predominance of resorptive processes involving osteoclasts over osteoblastogenesis. At the same time, a significant decrease in the level of VEGF suggests inhibition of angiogenesis in the bone tissue after chronic prednisone administration. The normalizing effect of vitamin D3 on mineral metabolism and the structural and functional state of bone tissue was demonstrated in the studied types of osteoporosis, as well as the positive effect of methylene bisphosphonate on the content of bone mineral components. The co-administration of vitamin D3 with methylene bisphosphonate provided an additive effect on the normalization of mineral metabolism, the structural and functional organization of bone tissue and the content of individual components of the RANKL/RANK /OPG cytokine system and the HIF-1? / VEGF. Scientific and technological development of obtaining the substance of a complex preparation, including vitamin D3, methylene bisphosphonate and calcium, for the treatment of bone tissue diseases, in particular osteoporosis, has been carried out. It is proved that the effectiveness of the drug and the shelf life of the active components of the drug depends on the technology for producing salt granules, vitamin-mineral mixture and the form of cholecalciferol used. High efficiency and bioavailability of cholecalciferol in the composition of the drug "Videin" makes it the most optimal form of vitamin D3 within a complex drug for the treatment of bone tissue diseases.
