The subject of the study is occlusion-stenotic atherosclerotic lesions of the vessels of the brain and cerebral aneurysms. Scientific novelty: For the first time, indications and contraindications for the use of surgical revascularization in patients with cerebral vascular pathology with the use of modern methods of studying cerebral hemodynamics have been established. Improved and implemented methods of creation of low-flow and high-flow extra-intracranial anastomoses (EС-IC) in patients with cerebral vascular pathology requiring surgical revascularization. For the first time in Ukraine revascularization microsurgical interventions were performed at intracranial aneurysms. For the first time the probability of using revascularization interventions with the use of combined (endovascular and microsurgical) methods of treatment is grounded. The effectiveness of methods of surgical revascularization in patients with cerebral vascular pathology on the basis of complex clinical and instrumental analysis has been investigated. The practical significance of the results of the study is to introduce surgical revascularization methods in patients with a variety of cerebral vascular pathologies, which include multiple occlusive and stenotic lesions of cerebral arteries, isolated stenosis and occlusive lesions of cerebral arteries in the intracranial divisions, and other stenosis cerebral arterial lesions. Practiced and advanced techniques of low-flow and high-flow extra-intracranial anastomoses are introduced. Based on the results of the study, the probability of revascularization with the use of combined methods is substantiated. Implementation of the work done. The diagnostic and therapeutic algorithm was implemented in the practical work of the Department of Urgent Vascular Neurosurgery of the Romodanov Neurosurgical Institute and it will be implemented in other neurosurgical departments of Ukrainian hospitals.
