The purpose of the work is to increase the effectiveness of treatment and to improve the quality of life of patients with breast cancer with an unfavorable prognosis by developing new methods of combined treatment. The direct results of treatment of patients with breast cancer with the use of standard and developed new methods of neoadjuvant polychemotherapy have been analyzed. A comparative analysis of intermediate results suggests a significant advantage of modified chemo-radiation therapy and selective intra-arterial polychemotherapy compared to standard systemic polychemotherapy. The method of treating edema forms of breast cancer has been developed, where a selective intraarterial route for the administration of chemotherapeutic drugs is used. The advantage of using this method in the number of complete (6.6% in the primary versus 0% in the control group) and partial regression of the tumor (37.5% in the main vs. 25.4% in the control group), and the amount of progression of the process (0% in the main group against 3.6% in the control). The developed technique allowed for radical surgical intervention to be performed by more than half of the patients in the main group due to the high rate of partial regression of the tumor. The developed method of treatment of patients with locally advanced breast cancer with the use of system-selective neoadjuvant polychemotherapy using magnetic therapy contributes to a significant increase in the objective response of the tumor to treatment (it allows to increase by 14,7% the cases of complete and partial regression of the primary tumor according to the criteria RECIST). Conducting neoadjuvant system-selective polychemotherapy with the use of magnetothermic allows to increase the percentage of conducting organo-preserving and by 5.4% of reconstructive-restorative surgical interventions by 13.2%. The inadequate efficacy of cytotoxic chemotherapy in hormone-dependent (ER + / PR +) tumors has led to clinical trials of neoadjuvant endocrinotherapy, which has recently developed as an alternative to the treatment of postmenopausal patients with ER-positive tumors of large size. The method of neoadjuvant hormonotherapy for patients with lung subtype of MMPHD is developed, which includes the appointment of aromatase inhibitors for up to 4 months. In patients with breast cancer after neoadjuvant polychemotherapy it is possible to conduct intraoperative detection of "watchdog" lymph nodes (efficiency of the procedure was 87.5%). The method of detection of "guard" lymph nodes prevents postoperative complications, reduces the term of treatment, improves the quality of life of patients.
