Purpose - To increase the efficiency of treatment of patients with oral cancer by developing functional and preserved approaches to comprehensive treatment. Complex treatment of 438 patients with oral cancer of ІІІ-ІV stages was performed: with the use of induction chemotherapy in the first stage before radiotherapy, in 101 simultaneous chemo-radiation therapy, and in 234 surgical interventions and postoperative simultaneous chemo-radial therapy. The analysis of direct results of treatment showed the benefits of using simultaneous chemo-radiation therapy in relation to induction chemo-radiation therapy. The complete response to the tumor is 9.8% more often (18.9% vs. 9.1%, p <0.05), the partial response is 25.5% (52.8 and 27.3%, p <0.05) respectively. The use of simultaneous chemo-ray therapy in the first stage of complex treatment of patients with oral cancer resulted in significantly better locoregional control and functional-preserving surgical intervention in 47.2% of patients, which is 19.9% ??more than during induction chemo-radiation therapy (27.3%), p <0.05. The remote 3 year survival did not show the benefits of one of them (32.7% and 28.2%, p> 0.05). Implementation of reconstructive-restorative operations allowed to restore the oral cavity function in 91% of patients within 10-14 days after surgery, and to achieve high indicators of functional rehabilitation of the oral cavity, namely: the full value of the diet 81.3%, the comprehensibility of the language 86.5%, nutrition on people 85,4%. The application of the surgical approach in the first stage of treating patients with resectable oral cancer of stages III-IV allowed to achieve a high overall 3-year survival in (62.7 ± 3.2)%, 5-year-old (55.4 ± 3.3 )% of patients.
